Job Description

Licensed Practical Nurse

We are seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our Recovery Station team as Nurse Coordinators (I, II, III) . The Nurse Coordinator is responsible for assessing participant health needs, screening for withdrawal or mental health issues, coordinating medical care, and ensuring safe, supportive recovery services. Acting as a team lead , the Nurse Coordinator will provide clinical oversight to EMTs and Recovery Specialists, while supporting continuity of care in collaboration with physicians, nurse practitioners, and other providers.

Primary Accountabilities & Responsibilities

  • Provide comprehensive participant care, including admission health assessments.
  • Accurately document all medical assessments, interventions, and outcomes.
  • Serve as the shift lead for medical operations and care delivery.
  • Monitor vital signs and conduct ongoing health evaluations.
  • Screen for withdrawal severity (e.g., CIWA assessments) and intoxication risk.
  • Conduct physical assessments: bruising, rashes, injuries, or bone deformities.
  • Administer breathalyzer tests as needed.
  • Screen participants for immediate psychiatric needs.
  • Supervise MAT induction in partnership with an RN, NP, or remotely with an MD.
  • Provide wound care (cleaning, antiseptic, covering, infection monitoring).
  • Administer and document medications accurately and promptly.
  • Monitor progress and determine whether participants can be safely treated in the facility.
  • Supervise TeleMAT induction to support patient treatment.

Team Support

  • Act as shift lead, supporting EMTs and Recovery Specialists.
  • Participate in shift hand-offs, reporting health-related concerns.
  • Provide training and guidance for Health Techs/Recovery Specialists.
  • Ensure safe, clean, and supportive working conditions.
  • Serve as backup to the Program Manager when needed.

Quality Assurance

  • Maintain accurate and up-to-date EHR records.
  • Ensure risk assessments, ER discharges, and medical clearances are uploaded daily.
  • Monitor for documentation accuracy, compliance, and confidentiality.

  • Ability to train, lead, and support a multidisciplinary team.
  • Strong crisis intervention and de-escalation skills.

Qualifications

  • Current New Mexico LPN license (required to legally practice).
  • CPR/First Aid/AED certification.
  • Valid New Mexico driver’s license and clean driving record.
  • Previous experience in substance use, mental health, or recovery settings preferred.
  • Demonstrated ability to take on increasing leadership and shift responsibility.
